All new patients should contact our friendly reception team who will be happy to answer any queries you may have and help book your initial consultation. Please note that it is our practice policy that fees payable for an initial consultation should be paid for in advance of attending for your appointment. For your convenience this may be done by telephone using either a credit or debit card.

Please note that all new patients visiting our practice should complete forms (i) and (ii).

Patients enquiring about braces or suffering with headaches will need to complete forms (i), (ii), (iii) and (iv).

Nervous patients should complete forms (i), (ii), (iii), (iv) and (v).

To return the forms electronically:

1. Download the required Word document to your Word program
2. Edit the document with your details
3. Save the document on your PC
4. Send an email to team@rdpholisitic.co.uk with completed forms as attached files.

To return the form by post:

1. Download the pdf document
2. Print document and fill in your details
3. Send the completed form back to us in the post:
