All new patients will be asked to complete some brief forms regarding their medical and dental history. All patients will need to complete forms (i) and (ii).

Patients who are enquiring about braces, or who are suffering from headaches should please also complete forms (iii) and (iv) and nervous patients should also complete form (v).

In addition to this all new patients are requested kindly to book an appointment by telephoning the surgery. It is a condition of the Practice that all new patients pay the consultation fee for their first consultation in advance of the appointment. For convenience this may be done by credit card over the telephone.

To return the forms electronically:

1. Download the required Word document to your Word program
2. Edit the document with your details
3. Save the document on your PC
4. Send an email to team@rdpholisitic.co.uk with completed forms as attached files.

To return the form by post:

2. Print document and fill in your details
